I've been using the Microsoft Intellimouse Explorer for Bluetooth. It's
right about as small as I can use and still be comfortable. However,

something is going wrong, or it just isn't handling the plethora of crud
that now all sits in the 2.4GHz band. I need a replacement.
I just tried the Microsoft Wireless Laser Mouse 8000, but the
combination of
a little smaller and ambidextrous makes it just a little uncomfortable.
There also seems to be something screwy about its fine precision. I
could
get used to it, but I could get used to getting punched in the head,
too.
I'd rather have something better.
Didn't Doug Engelbart make a large mouse some 45 years ago?
More seriously, have you considered a moderate-size trackball? I find
that the Mouse-Trak is quite satisfactory. Probably hard to get
nowadays, There is one new industrial-grade Mouse-Trak with PS2
connector on eBay at this moment.

I second the trackball idea. I'm 6'2" and have large-ish hands. I've been
using trackballs for over a decade.
I had the first version of the Kensington Expert Mouse (misnamed, it is a
trackball, really!) for 10 years before it crapped out and I loved it:

For the past five years or so, at home, I've been using the first version
of the Logitech TrackMan Wheel. It's very comfy. I see they have a
wireless model now:

Lastly, I had my employer (three jobs ago) buy me a now discontinued
TrackMan Marble F/X, which for me has been the ultimate pointer. It's been
about eight years and the thing looks like hell. I'm going to cry the day
it stops working because they're getting pretty expensive on E-bay.
