More seriously, have you considered a moderate-size trackball? I find
that the Mouse-Trak is quite satisfactory. Probably hard to get
nowadays, There is one new industrial-grade Mouse-Trak with PS2
connector on eBay at this moment.
I second the trackball idea. I'm 6'2" and have large-ish hands. I've been using trackballs for over a decade.

I tried a trackball, but I use software that requires too much composite pointer coordination (VLSI design software, 3D modeling software, etc.). Something which is a quick, circle-like gesture on a mouse is absolute murder on a trackball.

It's gotta be a mouse.

Hmmm. I've known a few artist-types over the years that preferred trackballs over mice solely because of the added precision. Note that they used the more 'hand' oriented trackballs, like the Kensington Expert Mouse, and not the little thumb-twitchy things. With a bigger, heavier ball you also get some measure of 'momentum' where you can zing that pointer all the way across a wide screen in one motion instead of having to lift your mouse up and move it when you hit the edge of your mousepad (or other mouse real estate.)

Personally, and in a more general sense, the reason I originally started using trackballs was for general comfort. I don't have to move my whole hand/wrist/arm to use it, just a thumb and my middle fingers most of the time.

I won't be so bold as to tell you you're wrong about your opinion :), but you really should give a giant trackball a try, something where you have to use your middle and ring finger to roll the ball. If it's for work you might even be able to get your employer to pay for it, so you won't feel bad when you have to pitch it or give it to someone you don't like.

I'm starting a job next month in a shop that loaded with artists and modelers. It'll be interesting to see what they all use (tablets maybe?)
