Results of actions to determine presence/absence of focus when file is opened

Thank you for the clarifying your message; yes, I would like to "catch the
culprit."

Yes, there are 2 methods of opening saved files within the "Recent Documents"
framework.  I use the icon method (as shown in my screen shot).  If the first
icon (which is highlighted by default) is what I want, I press the enter key to
open that spreadsheet.  If the spreadsheet I want is represented by an icon
further to the right, I use the arrow key to reach that icon and press enter to
open the spreadsheet associated with that particular icon.

The attached spreadsheet provides detailed answers to your questions.  If I am
missing something, please let me know and I will be happy to experiment
further.

Summary: a file saved the last time it was opened, opens with focus; if the
file was not saved the last time it was opened, it opens without focus.  This
is true irrespective of the method (Recent Documents/File Menu) and tool
(keyboard/mouse) used.  I am putting attention to actions taken in the previous
session because they affect focus in the current session.  Actions take in
current session will affect subsequent sessions.  Key word: saving.

Hopefully, this "experiment" will shed light towards a [permanent] solution.
