I am handling a few thousand domains with qs, SA, AV and qmail myself. No problem at all. With bayes and proper rbls I managed to get spam down by around 85% more than it was with plain SA. Use magic-smtpd/mailfront/recipent's patch/Bounce trim patch and some more. use them to your taste. It is qmail and no sir it does not SUCK. People might say that when they fail to appreciate the beauty of its modular design and its code. People (in India too) are handling multiple times that load on qmail.
