I am handling a few thousand domains with qs, SA, AV and 
qmail myself. No problem at all. With bayes and proper rbls
I managed to get spam down by around 85% more than
it was with plain SA. And I have many other things to do
than manage mails. The only problem I face(d) is due to 
some broken formmail.cgi.

Search for tarpitting. Use magic-smtpd/mailfront/recipent's
patch/Bounce trim patch and some more. use them to your taste.
For one customer with almost similar setup as yours I handle it
quite well with qmail.

It is qmail and no sir it does not SUCK. People might say that
when they fail to appreciate the beauty of its modular design and
its code. People (in India too) are handling multiple times that 
load on qmail.
