Two things you need to find out about this card are:
1. Is it s "Winmodem"? Winmodems are, to simplify a bit, cheap modems that
work ONLY with Windows. If that's what you have, no amount of help will get
your modem working with Linux; it just isn't possible. You have to look at
the modem documentation to figure this out.
2. Is it Plug-and-Play? If it is, you will need to use the Linux PnP tools
to configure it. I don't know the details of this, but there is a HowTo or
mini-HowTo on the topic (liik in /usr/doc on your system or at URL
http://metalab.unc.edu/LDP/HOWTO/HOWTO-INDEX-3.html#ss3.1 .

The use of /dev/modem is NOT your problem (based on your answer to #1
above). Something else is ... don't yet know what, but my replies above
suggest several possibilities.
