Probably a "winmodem," especially if it's PCI. IOW, a "paperweight."
I'd ditch that USR V.90 internal and either get an ISA internal
modem, or an external modem. There is NO support at this time for
'winmodems' and AFAIK, there are only TWO PCI modems that are not
WinModems, HSP modems or some other "software" modem. Unfortunately,
USR doesn't make either of them.

domain is cvtci.com.ar
hostname is the name you gave your machine at install, plus your
ISP's domain. For example, mine is slave1.chattanooga.net because my
ISP is chattanooga.net, and my machine name is slave1.
My advice is to ditch that USR for a non-WinModem. I almost guarantee
you'll have no problems if you get an external modem! Plus, if it
hangs for some reason, you don't have to reboot the machine to reset
the modem. ;-)
