Personally remove the fan by unscrewing it where possible, then use a
paintbrush or shaving brush to dislodge the dust.  (no paint or cream
has ever been on the brush of course!)

If it looks particularly bad I'd do this outside.

Then re-goop the heatsink and reassemble
