I think for the 3c509, all you need is:

            option option-128     e4:45:74:68:00:00;
            option option-129     "NIC=3c509";


Make sure you have option-128 specified exactly as i've
shown.

The other thing you MUST do is make sure that you've disabled
plug-n-plan on the card.  You will need the 3com utility to
do that.
