Brian,
Number one - when booted, check that the OS 9 "System Folder" is
actually "blessed". It should have a "9" on the "System Folder"
folder.
If it doesn't you need to "bless" it. Lots of sites on the web for
doing so, for an example try:

etc. etc. etc. That should do you for a bit to get your 9 System blessed. ;)
PS: You might want to shut down, disconnect ALL peripherals, restart
and try the blessing procedures as you don't mention your whole Mac
situation. ;)
The "floppy icon with a question mark" means your OS 9 "System
Folder" folder is NOT blessed. Or you'll need to reinstall your OS 9
OS. (Check the options when doing so.)
