Admittedly, the pdf’s I frequently access are not as large as you’ve mentioned, 
although, just for investigative purposes, I opened a pdf I used to access on 
Open Directory Administration within OS X Servers.  This pdf is 295 pages long 
with many or most of the elements you mentioned in the academic articles you 
are required to read.  Upon opening it, VO announced that the document was 295 
pages long and landed me on the pdf Content area.  I interacted twice and began 
reading with no issues.  Of course, I didn’t take the time to read all 295 
pages but whether I went line by line or VO-a to start reading from where I 
was, VO read the pdf just fine.  I realize that there are issues within Preview 
and it’s handling of certain tables and such within the document, but the 
headings, sub-headings and other textual content was read normally.  Images 
and/or figure 2, for example, were not read for obvious reasons.  I guess what 
I’m saying is that I don’t believe that the size of the file is the issue.  I’m 
not sure what the issue is, nor am I trying to say that there’s no problems at 
all with Preview, but, I appear to be able to use Preview just fine with most 
pdfs.
