I've tried all the combinations  but none of them work unless a monitor is 
physically connected.
Perhaps some sort of vga simulation device would work but at some point it is 
just easier to just buy a monitor than to keep wasting money trying to find 
ways to avoid one.
As has been pointed out monitors are quite cheap these days so it shouldn't be 
a hardship to buy one, I bought one from Tigerdirect for $69.  Just a pain.
