Here is a thought. If the i p timing and valve timing are off from each
other by 1 complete revolution of the crank, (and + -18degrees) I think
the described conditions could exist.

Using a TDC indicator of some sort ( a stick, light pressure/vacuum
gauge) will give you true crank position and the marks up top are
what they are. I would say almost no likelyhood of cam/sprocket
misalignment but there's a first time for everything.
