ProxyPassReverse does a literal string prefix replacement on whatever is
in the Location header.

If what you have specified to replace, and the Location returned by the
backend server are not the same, no replace will happen (obviously, as
there is no prefix match). Make 100% sure that the URL the backend
server thinks it is, and the URL that you have configured in
ProxyPassReverse are the same - otherwise garbage in, garbage out.
