When the British coroner accuses the NHS of "a gross failure to provide
basic medical attention," there's no problem in communication. Whatever
problem one finds in this thread comes from Ant McWatt blaming a private company
nurse working under NHS auspices for the woman's death when in fact a NHS 
hospital
failed to diagnose the woman's illness on two previous visits. I found the most
accurate version of the story in terms of journalistic professionalism and 
accuracy 
from the Scotsman newspaper whose headline was:

"Coroner Slams 'Failures' That Led to Woman's Death"

Followed by the lead sentence;

"A woman's life could have been saved on at least two occasions but for a "gross
failure to provide basic medical attention" by hospital staff and an 
out-of-hours
primary care service, an inquest found yesterday." 

Incidentally, the Scotsman cites a number of other NHS failures which I'll
gladly share if need be.
