To answer the original question, the Minnesota State Highway Patrol, under the Department of Public Safety, has a form on their website (https://www.dps.state.mn.us/patrol/unsafe/report.htm) Their statement is as follows:
"Use this form to report any unsafe driving incidents that you may have witnessed. An example would be observing a vehicle swerving in and out of traffic, traveling at a high rate of speed, or any other unsafe maneuvers. Any and all information you can give us about the vehicle(s) involved would be of great benefit to us. As a result of your complaint, the registered owner will be sent a letter describing the complaint submitted. Due to the large volume of complaints received by this office, we are unable to provide you with an individual disposition of your complaint."
