Here's my current frontend/backend system:
- Athlon64 3200+ (Socket 754)
- Sis 755 motherboard (with heatsink, no fan, on
Northbridge)
- Zalman CNPS 7000Cu (pure copper)
- Seasonic Super Tornado PSU
- eVGA GeForce 5200FX video card (fanless)
- 2x PVR-x50 (one 250, one 350)
- Antec SLK3000B case
- DVD-ROM
- 3x Maxtor 250GB (7200 RPM) IDE drives

This PC is so quiet that I can't hear it when I'm
father than two feet away from it, most of the time. 
My XBox, my desktop PC (which has similar components,
except for a Northbridge fan) and my laptop are all
louder than it.  The only time that I can hear it is
when the case starts vibrating, I suspect from the
three hard drives seeking so quickly.  I'll probably
be adding some acoustic foam to the case to help out
with that.

Putting together a very quiet system, unfortunately,
costs a fair amount of money.  Don't ask how much I
spent on this rig.  :)
