Neck and back problems.  You may want to visit your dentist.  You are really straining your jaw, and that affects other muscles.  S/He may be able to help you with exercises to stop grinding, and a bite plate for night time use.

You may also want to make sure that you are not grinding your teeth.  It is amazing how many problems that will cause.

  big time in a car accident - funny thing - after months of physiotherapy &
  pills and pins all that worked was using a chair at work with no wheels - no
  pressure tilt on the back rest etc. My physiotherapist recommended I try it
  as he believes people constantly and subconsciously tighten and relax muscle
  groups to counteract a chair with wheels tendency to slide back. Anyhow that
  and keeping my chin tucked in and not extended out like Jay Leno worked for
  me.
