Friend of mine used to work for a now-defunct datacenter company. One day, they had a fire in one of the cabinets for their giant UPS system. Per the fire department, they had to hit the big red switch. Unfortunately, their transfer switch was part of the UPS system, so when they EPO'ed the UPS it took the load down with it. So they get a separate, manually-operated transfer switch installed. That way, even if the UPS melts down to slag, they can cut over to line or generator in the meantime. Within a week of installation, there was an electrical fire in the new transfer switch.
