I would suggest, from professional experience with the costs of printing and
packaging, that it is considerably easier and less expensive to simply print
the licenses on pages of the book (whether that means adding pages or just
adjusting your layout to make room), compared to the cost of a separate
license sheet that has to be tucked in loose and thus may require that the
whole thing be shrinkwrapped, for example.
