What I'm curious about is why you don't just do something like this?

It's a rough version and I'd want something that had some more time put into it. But its perfectly usable... could probably use some explanation paragraphs..

Also, notice the copyright section (#15) I've noticed that a lot of companies don't seem to be filling that in, or have I mistaken the proper usage for that?
