I wouldn't think poorly configured proxy servers. Many countries use them
to limit the type of Internet information into the country - Singapore (the
most restrictive nation in the world concerning Internet access controls
the whole country's access to the Internet by proxy servers), the UAE.
Saudi Arabia, Iran, and other middle-eastern countries all use proxy
servers to control what their people receive from the Internet.

There is an entire mailing list devoted to this subject
([EMAIL PROTECTED]) and web site
(http://www.ijs.co.nz/index.htm). When I was the administrator of the
Accmail mailing list, we discussed e-mail only methods to bypass Saudi
proxy servers. It turned out to be a cat-an-mouse game between ISPs and
users. As soon as one method worked, it was promptly blocked by the ISP.
The ISPs even resorted to blocking specific e-mail addresses. The only sure
fire method was to use the nym server.

Poorly configured mail servers susceptible to port 25 hacking I can
believe. However, most ISPs and system administrators try to "plug" these
holes as fast as they are found. There probably are some "rogue" servers
out there that spammers use with abandon, but most of the spam I see is
from mass mailer software that configures it own SMTP server.
