I find it easy to forget that I need to have the shutter button partway 
depressed for a second prior to taking a picture if I want SR to work.
I've had a few shots where I thought SR wasn't working, until I noticed that 
it was mostly with manual focus lenses, where I tend to not depress the 
shutter button at all until the moment of exposure.
The AF button does not invoke SR (it should, I think) when used, so if you 
tend to use the AF button to focus and the shutter button to shoot, you 
might be bypassing the SR mechanism.
