Short version:  Use a good charger, and the right rechargeable batteries.

I've always used rechargeables in my *ist-D.  About 400-450 shots on a set
of 1600mAH Ray-o-Vacs was a typical number before I had to switch to the
other set (I had the battery grip, set up to use batteries in the grip first).
With a good charger (a Maha C401FS, in my case) these gave me no problems
for around four years of use.  When it came time to replace them, though,
I had nothing but problems with the Energiser 2500mAH batteries.  Nowadays
I use Eneloops, which are fine under almost all circumstances.  The one
time there is a problem is if I'm trying to donload a few images over the
USB cable, rather than hunting around for my CF card reader; If I've got
the grip mounted the camera always shows "no battery charge left" if the
USB cable is connected, and often only shows half charge with no grip. I
keep a set of Ray-o-Vac hybrids near the computer for use while downoading.
