I don't think the original UW-IMAP supports Maildirs, although it's been awhile since I last checked, maybe the new ones already do. Just so you know, imap-4.5- 3mdir4 is kinda old. I think that's the same patched RPM I used before before pa when I first forayed into Maildirs with qmail. Then I stumbled across Courier which bragged so many things over UW-IMAP so I jumped on, tried it out, got it to work, and have been happy since. It's also so much more feature- packed, and that's on top of the core services which I think it does better.
