The only thing possible is if you use composition mode where the same file is overwritten after each shot, but in the case you was using this you would have big exclamation marks in all four corners of the preview on screen. Besides that it is really hard to what might have caused it with out doing some further testing with the computer and camera.
I know I might make some annoyed by saying this and I hope I do not step on any regulations for this mailing list but once again I must underline that at the moment there is no DSLR solution that is specifically made for this kind of work. If you want true tethered equipment then it's medium format camera back you should look in to. If you want a hybrid, then the same come for that. Phase One P series, Imacon iXpress, Sinar M model or Leaf portable/DP67 which all have a stabile firewire interface but also some kind of storage solution, onboard like a DSLR or on external unit.
