I haven't gotten around to dismatled the CV / Driveshaft this time but.... The first failure seems to be caused by the Driveshft "pulling" the centre of the CV joint out of its casing.
The centre was still attached to the Driveshaft and the ball-bearings were left behind in CV casing amongst the grease.
