Chad, I have quite a list of sites like this as I've also bee looking into self-hosted project management/tracking systems. Mostly I like Basecamp and ActiveCollab but they kill me in monthly charges once you get some real project information stored into them. And I've not decided to purchase ActiveCollab just yet.
If you are just looking for more a help desk type platform I would suggest looking into Mantis (http://www.mantisbt.org/). It's a mature PHP based MySQL bug tracking system similar to bugzilla. My problem with hosting bugzilla has always been the needed Perl are hard to manage in a hosting environment. If you are looking for something a little more like Project Management check this page which is a list of Basecamp alternatives. Though many solutions mentioned are the same basic model: centrally-hosted with month charges based on project sizes. http://pm-sherpa.com/features/basecamp-alternatives/ Two that I've started using recently on project management are Project Pier http://www.projectpier.org/ - PP is a cousin or sibling to ActiveCollab. There was a code split some versions back when AC when commercial. Also Collabtive http://collabtive.o-dyn.de/ A very well done Project Management tool. Very stylish and AJAX-y UI. Though less feature rich than Project Pier.
