Great suggestions from the group, lots of choices and having done this search many times over I know how exhausting it can be.
I don't see anyone who has recommended RT (Request Tracker) so I'll throw that one in the ring. I've used in multiple (very different) scenarios and although a bit of a pain to get set up, once running it's a great system. Excellent threading of issues, email integration, etc. When I used it the interface was atrocious but it looks as if that has been updated quite a bit (and I'm glad to see it's still in very active development by BestPractical). http://bestpractical.com/rt/features.html http://blog.bestpractical.com/2008/07/today-were-rele.html#screenshots It is open source (perl) but you can get support from BestPractical (though I can't speak particularly to that). I'll throw Roundup out there too.
