Tarn is correct about it being TPOS and everything else looks about right. The one thing I noted from your Tag and Rename Screen is there appears to be blank spaces in the 1 / 2 TPOS field. Compare it to the way the track numbers look (1/8).
I'm not sure if this would be problematic for Slimserver's code, but if I were you I might try changing it to 1/2 to see if it resolves the situation.
