In my case, it is usually someone "testing" the multimegawatt
building-wide UPS in dumb ways.  ("Oh, the On/Bypass/Off switch is
sticky... oops, we just killed power to the entire building... quick,
turn to Bypass so they get a nice spike as everything comes back on!") 
I have learned to not even trust UPS's and generators.  (Ah, but we had
a neato Liebert power conditioner at one point that kept chugging along
after a phase dropped from city power.. it somehow managed to fake an
entire phase and not burp at all.)

That and IBM made some poopy drives for a while: I'm glad they got out
of the drive biz and I'm glad all the replacements were under
warranty.

And, yes, my backup disk is a USB drive that is unmounted, unplugged
from USB, powered off, and unplugged from the wall when I am not using
it. :)

Whatever your level of paranoia is, it is never enough to be safe. 
Gremlins will find you regardless of what you do.
