I've been struggling with this for a while, I have all my cd's ripped to FLAC and keep a mirror of all the tracks in MP3 for iTunes, including the cover.jpg files for each album. I was about to manually add all the cover art to the tracks in iTunes when I found this:
http://itsfv.sourceforge.net/ It seems to be the answer I've been looking for. You need to do some tweaking of the settings (changing the default folder.jpg to cover.jpg, among other things). Be very careful, this program has lots of options and not a lot of warnings, backups would be prudent. I'm running this for the first time now...
