I don't know how the files I'm testing with were created, I'm afraid. I've got the player audio settings set as follows: ReplayGain off, volume control output level fixed at 100%, I don't have LAME installed, so the options to rate limit are greyed out (set to No limit). Was there something I needed to do with file type settings on the server side?
