Git/hg/svn blame should tell when and who has last changed a line in file. It should be quite easy to gather the relevant data from output of the blame command. I think that you would also get much better tool support if you would use version control system for the tracking task. For example gitstats can generate very nice graphs of 'who has done what' and how many lines have changed etc..
