I had that happen when spikes from different polarity rails touched inside a 
tie.   It gets pretty obvious, as you indicated, by the smoke rising from 
the spot...

The strangest thing I ever saw was a problem module that got diagnosed by 
the wisp of
smoke rising between a spike and the rail. Apparently something conductive 
was buried in
the Homasote, creating a dead short.
