Hi,
For me it is voice controlled. I don't need a phone or a computer to use it. 
Which I do for skype and the other applications you stated. And I can use it as 
a wireless intercom or to talk and message across the country or world without 
picking up a phone or taking my focus away from whatever I am doing on my PC.
I just had a long conversation with the people on the Echo team at Amazon. They 
are telling me that the new Echo show which is coming out in June will require 
sighted assistance to set up. And because the screen is not a touch screen as 
in a tablet but more of a display to include additional info to supplement the 
voice interaction, there is no plans to include any text to speech option on 
the new touch screen which is part of the new Echo show.
I was a bit disappointed because I was looking forward to getting a pair of the 
new shows and using the video call features. However, since I'm having so much 
fun with the audio call and message feature on the dot, I just decided to 
upgrade to a pair of Echoes while they are on sale at Amazon for $149 US$ until 
tomorrow.
For me, this message and call feature allows me to be social but still be more 
private than skype and Zello and whatsup etc. Because I don't have to join 
silly groups or advertise or knowing me, hide my online status while I'm 
working.

Here's another thought. I wonder  if one has to have Wi-Fi calling enabled to 
use this feature. If so, then that's probably why it's not working for me.

This does work outside the US.  I talked to a friend from Australia using
Alexa.  It really is not a huge deal.  It is yet another bit of technology
in the tool box.  It is simply a hands free method of quick messaging and
making calls.

in the US, Germany and I can't remember if it is Australia and/or the UK. I am 
in Canada and officially you can't buy an Echo here and the Alexa app is not

store.
The app name is Alexa.
You set it up by filling out the information it asks for.
If that isn't enough to get you going, perhaps you need to find someone who can 
walk you through it in person.

The app is required but the hardware is not required. The Show is not
available until June so we won't know how accessible that will be. It
has better speakers, and htat's about all I think it's good for unless
you can look at a 7" screen. Some have commented about difficulty with
setting up the app. It's hard even with Voiceover off, very sluggish.
But I found that if you totally restart the phone you'll be able to
click on the links with VO. Just interact slowly with it because it's
extremely sluggish at best. Otherwise, what a fun feature this is.

I just updated the app on my iPhone 6 running the latest iOS and find that I 
cannot get it to work with voice-over. I can read the screen but cannot click

does the person you were trying to call have the Alexa app installed?
I think it only works with people who also have the app.
And yes, I suspect they have to also do the setup.

I set up the calling feature, verified my phone number and put in the code 
which is sent. I think each other person you wish to call who is an Echo device 
user has to go through this verification and set up process in order for it to 
interact successfully with Alexa.
I am new at this part of this however. I tried to call someone and she said 
this person wasn't in my contacts.

I have used both the calling and messaging on my Echo Dot. It works very well. 
I am now considering a pair of Echo Shows. I have been waiting to see if anyone 
can give me a handle on the accessibility of the touch screen. But the 
demonstrations I have seen sound pretty good. And it works so well on the dot 
I’m already hooked.
