For 9.1.04 , there is a roll back utility to roll back platform upgrade
(AR, Atrium Core, Atrium Integrator) which also roll backs control table
mentioned by Doug. Please check with BMC support for this utility. You will
have to run this utility and then run upgrade again.

I believe your mid-tier version is also 7.6.04 SP2 which is quite old ,
where we did observed few caught exceptions problems.
Best bet is to upgrade mid-tier to latest version apply latest hotfix on
top of it..either 7.6.04 SP5 or 8.1 SP1.
It doesn't take much time to upgrade only
