Did you try restarting BlackBerry Synchronization Service and BlackBerry Policy
Service?
Usually, those services get stuck from time to time.
After restarting the above mentioned services, reset the activation process.

There is no 100% guaranteed way to check the provisioning on the SIM card
except through RIM's provisioning website which only carrier's team will have
access to.

Two more things you can try,
1. Uninstalling MS05051 hotfix which has changes to RPC that affects
communications between BES and Exchange.
2. Recreating MAPI profile.

I would suggest you try configuring a proxy authentication under MDS
