The *** Error: Bone access denied [...] caught my eye, does anyone know
what it is? I have never seen it before?

Never seen it in TF2, but in CSS is meant that a core file being accessed
didn't match the file on the server. As I stated to the OP, if one / two
people are getting it, the problem is on their end. If multiple people get
it, then its cause to believe that there is corruption server side.

That almost looks like a generic connection problem. The in-packets
being large seems to be justified by the choke spiking and then dropping
over and over. Is this only happening during map changes or in the
middle of the round?

More specific : See if you can get someone to send you a screenshot of
the console when these lag outs / crashes occur, or pull that log.
Reason I ask is that it looks to be another issue with Netchannel overflow.
-Things to look for. Are you running any custom sounds in server?

I've been getting reports of players locking up or lagging out in TF2. I
finally got someone to send me a screenshot while the problem was occurring.
