Never seen it in TF2, but in CSS is meant that a core file being accessed didn't match the file on the server. As I stated to the OP, if one / two people are getting it, the problem is on their end. If multiple people get it, then its cause to believe that there is corruption server side.

Good Morning, More specific : See if you can get someone to send you a screenshot of the console when these lag outs / crashes occur, or pull that log. Reason I ask is that it looks to be another issue with Netchannel overflow. -Things to look for. Are you running any custom sounds in server?
