That's because you're in full screen mode. The menu bar doesn't show in full
screen mode. Press Control+Command+F to switch to single view mode and the menu
bar will be available. Either mode works fine and doesn't affect the operation
of the VM - but full screen comes at the cost of the menu
What I do is always use the Virtual Machine in full screen mode -control
command and enter. If I want to return to the Mac all I have to do is
press control command enter again and then command tab will take me away
from the Fusion Window.

You'll need to remap the insert key, since caps lock won't work, as you've
already discovered. Try downloading sharp keys and you can remap the insert key
to the grave accent.
