There is one thing to take into consideration when permanently attatching a K to M42 adaptor on a M42 lens. The genuine Pentax adaptor when mounted on a K body is slightly recessed. This is good if you want a tight mount when screwing a lens into the adaptor when mounted on the body. The generic

Yes, I'd expect that. There are a lot of strange things when it comes to cameras. Incorrectly film loading (then shooting 36 exp. on one frame:), thumb over the lens, you name it (and you see it). It doesn't happen that way with a VCR, people seem to be able to put in a cassette easy enough.
