The way I read man ntpd (on Debian wheezy), we could (should?) replace
ntpdate by
ntpd -q; and if we are going to run ntpd then ntpdate is unnecessary anyway.
If we have (or are going to have) ntpd, then we should simply skip
/etc/network/if-up.d/ntpdate;
seeing how that depends on NTPSERVERS

Oh, you said that you have two icons; presumably the other one is from a
previously mounted DVD? Perhaps you ejected one using the eject button
on the drive? On 14.04 that would not automatically clean up the mounts
(bug 1281110). Does that also happen with the same DVD right after a
clean boot?
What ever the problem was, it is now fixed. Must have been fixed in one of the
updates that were installed since the problem was reported.

Various sources online suggest this bug is fixed in Network Manager
9.8.8.9 (Ubuntu is currently on 9.8.8.8) but I haven't verified this
myself. The bugzilla report in Gnome is closed for this reason.
