To assess sensitivity of touch screen on iDevice simply place your finger
very, very lightly on the screen. Then increasingly place more of the fleshy
part of your finger on the screen. Do this repeatedly. In very quick time
you will get

The best advice is to relax. Keep your fingers relaxed, not stiff. Slightly
spread the two fingers
during the call, you can hang up by
double-tapping the Power button.

By your wording, I would guess you are trying to tap too hard.
The key is consistency, not firmness. In fact, a lighter touch works better.

I often find it difficult to perform the 2-finger double tap gesture
for picking up or disconnecting calls.
It's hard to describe the problem, but I just struggle to generate
the force that is required to effectively perform this gesture in

As a result, when I take the phone away
to do
any voiceover commands works best.

Rahul, Try taking the phone away from your ear before you do the double tap.
VoiceOver commands do not work all that well when the phone is close to your
ear. If they did, you would likely be doing all kinds of things with your
face against the phone
starting and stopping the music playing.
