It has happened to me as well when the triple click command is active.
Sometimes I turn off the triple click and go through menu to turn VoiceOver on
in accessibility. Seems to solve the problem.

Yes you would be correct that I have usable vision... I am hoping that the
Apple genius will be able to see the issue that I am having with my original 4s
I could not re - produce the issue... As many times as I turned VO off it
worked fine
