Have you tried asking to speak to the doctor's nurse or secretary to ask
them to fax or send you a copy? Or to make you a copy so that you can
pick it up? Leaving messages for the doctor never gets me anywhere.
Maybe you already tried that, I don't know. But it seems impossible to
get a doctor on the phone, even when you haven't fired them. [grin]
