The business office of the drs around here take care of those requests.
If they have it in writing (if you write them a note or mail them a
letter, they are required to respond in a certain number of days. You
own your files, you paid for them, or the ins co paid for them on your
behalf. It seems to me like sometimes they don't want to give us the
numbers, because they believe we are not capable of knowing what they
mean, and some are even afraid that we might look something up on the
internet and get the "wrong" idea.
Maybe you could move up the appt, or write them, keeping printed
records of your doings, not just a computer record. Computer records of
a letter are not always valid in court, so print an extra copy and date
it with the date you mailed it. Then call and check to see if they got
it. Let the office know you are keeping records, that you are writing
everything down- that can make them skittish, and sometimes they will
give you what you want. If they refuse, you can say "Are you refusing
to send me my files, that I legally own?" and they will think about it
a little more. If they says yes, you can write that down, and they can
get into trouble, moreover, they will know it.
My doc's office has me address an envelope when I go it so they can
mail me the lab results as soon as they come in. That is the standard
for all the drs. The Park Nicollet comglomerate also mails nearly
everything out, whether you have an appt or not, the results go out by
mail.

Have you tried asking to speak to the doctor's nurse or secretary to
ask them to fax or send you a copy? Or to make you a copy so that you
can pick it up? Leaving messages for the doctor never gets me
anywhere. Maybe you already tried that, I don't know. But it seems
impossible to get a doctor on the phone, even when you haven't fired
them. [grin]
