actually the FWD ITAD doesn't seem to be working at all, but the screaming 
monkeys do work.

To make sure I understand your deployment. your server has a public IP address 
(no firewall in front of it)? I wouldn't recommend that, but if it is public 
then you need to make sure:

1. NAT TRAVERSAL is ON
2. Server behind NAT is OFF

If you installed from ISO, make sure iptables is not running for any reason.

service iptables off
service ip6tables off

If the server is not behind NAT, then remove any Intranet Subnets it is not

I've explained some of this before, but it's been deleted in the replies.
I have heard some AA prompts on at least one test number, but I cannot find
it now, so I can't reproduce it. I am able to hear wav files played when I
call into a conference I set up. This server was built from CD. I'm
confident the wav files are all there. It seems to be only the VM prompts
that don't play either when I call in to get VM (ext. 101), or when I call
another number and get its voice mail. When that happens, the server just
starts recording without generating any prompt.

I cannot do all of your suggestions. The server is on the Internet in
Kansas. I'm working from Houston behind a NAT'ed home router. But I did
capture SIP logs last night as Scptt suggested and put them in a zip file
here so someone can look at them with sipviewer:

I have all the DNS entries required created. I don't think it's a codec
issue, because I am able to leave voice mail, make and receive calls from
other extensions, and participate in a conference.

You might be able to answer another questions for me. I thought I saw some
numbers that can be used for echo tests, and others for other types of
tests, but I can't find those now. Do you know where I can find a list of
such numbers?

I appreciate your help. I will keep looking until I resolve this. I'm about
ready to rebuild. But it would be better to understand what's wrong. If you
have any other ideas, please let me know.
