Sincerely, I think you have a DB issue.

This:

delete from phone_group where phone_id = 12;
delete from line where line_id = 15;
delete from phone where phone_id = 12;

And this:

org.sipfoundry.sipxconfig.phone.polycom.PolycomPhone incompatible with
org.sipfoundry.sipxconfig.phone.cisco.CiscoPhone

Tells me that a table is hosed. This is unusual in that version. If
you have a backup from when it was not hosed, you might consider that
as your way out of this mess.

Additionally, do not attempt to upgrade it, I don't think it would be
a good idea.

You might do an export of users/phones that are still there, since
they will import into a new system, but there are a few things you'd
have to recreate manually so you need to make sure you get everything.

To prove my point, in a few days time you will find if your table is
indeed corrupt or broken, that you will not be able to make subtle
changes (like the description) of any phone or add/delete a line from
a phone, etc. to the point where nothing can be done with phones at
all.

I've been through this a few times with 4.2, but never with 4.0. Had
any abrupt power or related issues that would cause a systems problem?
Were you in sipxconfig doing something that seemed to take forever and
pulled a "Tony" and restarted services because you were impatient?
(that would do it too, as I learned if done at the wrong moment).

At least even if it is a rebuild, it's not impossible to recover from.
