I am using a D-Link DIR-632 with DD-WRT v24 firmware. I would think its unlikely the phones don't support REFER messages since transfers and placing calls on hold works correctly internally and for outbound calls. I am thinking its definitely a firewall configuration issue but am not sure what else to do since I am forwarding port 5080 to the SIP server as mentioned before.
