I am not aware that the DD-WRT supports outbound symmetrical nat (I
recall considering that some time ago before resorting to pfsense on
an atom based appliance to tackle this).

I think you need to find out from their community if it is supported,
and if so, how to do so. If not, moving to a firewall solution that
will handle NAT in a way thats compatible with sipx would be another
option.
