That might have changed since they brought in a different format for spotlight. Not so much "what's wrong" as "what's lacking" for me. Mailsmith has unrivalled applescript support (useful for using Dictate to control it); the cleanest interface; filtering systems that knock the others out of the water; highly complex search criteria [Mail is fast but often not precise enough]; absolutely unrivalled text manipulation; won't render HTML so is the most secure against all kinds of sneaky invasions via 'nich' text; conforms absolutely to industry standards; terrifying levels of support especially via the Mailsmith list; you can attach scripts to menus and actions...it's dizzying what it can do, frankly. and stuff like that. Mail is quite mature now (I remember the early days -- are you serious you didn't have problems? you must be the only person on the planet.
